Refer a client

For physicians, therapists, teachers, case managers and family members. We accept referrals by phone, email and fax.

How to refer

  1. Call (206) 796-4797 (Monday to Friday, 9 AM to 5 PM) or email admin@theevergreenpathways.com with your name, organisation, phone number, and the family's name and phone number. Please confirm the family has agreed to be contacted.
  2. Keep diagnoses and clinical details out of email. After we receive the referral, we will tell you how to send records, or you can fax them to (440) 332-2093.
  3. We contact the family within one business day, check coverage, and schedule an assessment with a Board Certified Behavior Analyst.

Who we serve

  • Ages 2 to 22
  • In-home and community-based services, with telehealth where it fits
  • King, Pierce, Snohomish, Thurston and Kitsap counties
  • Apple Health (Medicaid) fee-for-service; other plans checked case by case
