Consumer health data privacy policy
Effective October 15, 2026. For people in Washington, under the My Health My Data Act.
This policy covers health-related information that Evergreen Pathways LLC may receive from people in Washington before they become clients, for example when a parent calls or emails us after visiting this website. Once you are a client, your health records are protected by HIPAA and described in our Notice of Privacy Practices. Everything else about this website is covered by our Website Privacy Policy.
What we collect and why
This website has no forms, so it collects no consumer health data itself. If you contact us by phone or email, we may receive your name, phone number, email address, your city, the services you are interested in, and anything you choose to tell us. Because our services are behavioral health services, this may show that you or your child is seeking care. We use it only to reply to you, check coverage, schedule an intake and review a referral.
Where it comes from
Directly from you, or from a referral source who sends us your contact details with your permission.
Who we share it with
We do not sell consumer health data. We share it only with the companies that deliver and store our email and host this website, and with government agencies or others only when the law requires. We do not share it with advertisers or data brokers.
Your rights
- Know: ask whether we hold your consumer health data and which third parties we shared it with.
- Access: ask for a copy of what we hold about you.
- Withdraw consent: tell us to stop using the information you gave us.
- Delete: ask us to delete it. We will also ask the companies above to delete it.
Contact us using the details below. We will respond within 45 days. If we need more time, we may extend that by 45 days and will tell you why.
If we say no
If we deny your request, you can appeal by replying to us with the word "Appeal" and your reason. We will answer within 45 days. If you are still not satisfied, you can file a complaint with the Washington State Attorney General at atg.wa.gov/file-complaint.
